Population health concerns the health of all and takes into consideration their healthcare disparities. The nurse working in population health takes into consideration the differences in social determinants of health. In order to work in population health, one must have a certain required set of skills. They must have great communication skills, good critical thinking, readily equipped to work in change of demand settings, and the ability to work with interdisciplinary teams.

What is the nurse leader’s role in creating a culture that brings into play the social determinants of health?

The nurse’s leader’s role in creating a culture that brings into play the social determinants of health is to make sure all determinants are taken into considerations. In order to improve the quality of patient care and promote healthcare equity the nurse must analyze the social determinants of population health. In an article stated by Breanna Lanthrop, it is stated that nurses are patient advocates, patient care expertise, and equipped to lead the nation in strategies to promote health equity. In addition, the author goes on to state that nurses can embrace the leadership through the use of interdisciplinary collaboration, advocacy, political involvements, and community partnerships”. In doing so, the nurse is creating a culture that bring into play the social determinants of health.

What challenges are found in your geographic region that require new ways of providing health care?

Bring data from the county health rankings to help identify your geographic regions challenges. I live in Tenafly which is in Bergen County, New Jersey. The county health rankings showed that my county ranked number 3 for the overall health outcomes. Factors taken into consideration were length of life, quality of life, health factors, health behaviors, clinical care, social and economic factors, and physical environment. The worst ranking shown was physical environment which was ranked 11 out of 21. The county stayed within the errors margins for sever housing problems, driving alone to work, and long commute. In addition, it was shown that drinking water violations do exist in Bergen County.

For example, the CAFO is a regulation that addresses facility management and pollution. In addition, the conservation reserve enhancement program (CREP) is a federal state partnership that pays landowners an annual rate to remove “erodible areas” (County health rankings, 2019).

What was one new takeaway did you find in reading this paper? One new take way I found in reading this paper was the role that nurses play in population health. Nurses who specialize in population health are the ones that are out in the community’s advocating for change and trying to improve the overall quality of health. Through the participation of nurses, the promotion for healthcare equity will be readily achieved. The role of the population health nurse impacts the patient, but most importantly, the overall community. Nurses are a major contribution to improving the healthcare status of areas, and have been known to treat and aide patients to their well being in their homes and at their bedside. This article really put emphasis on the impact that nurses play within the community. Not only in the health field but also in policy making, and patient advocacy.
