Patient safety is singularly the most important item for any healthcare facility. Although facilities push for patient safety, medication errors are high on the list of safety concerns that happen several times daily worldwide ranging from non-life threatening to the demise of patientsThe purpose of this assignment is to discuss some of the reasons why medication errors occur and steps that can be used to decrease the frequency of this problem.
