The healthcare industry is a complex, innovative, and high risk. A lapse in the system or a misunderstanding of the way a system operates can affect clinical outcomes (Johnson, Miller, & Horowitz, 2008). The healthcare system is always changing; the idea is to provide safe and excellent quality of care to patients at lower cost. One of the top challenges for leadership is dealing with rapid change while leading more complex organizations (Marquis & Huston, 2017). The facility I work is a 72-bed long-term acute care hospital. Most patients admitted for wound care, hyperbaric oxygen treatments, long-term antibiotic therapy, and physical and occupational therapy. It’s important to note that the majority of these patients have multiple comorbidities and their medication administration records are extensive. Patient safety and positive outcomes are of utmost importance to the hospital as well as decreasing cost of care which brings a significant problem: increased nurse to patient ratio related to reduced staffing and poor employee retention. ……………..

Oganizations are run under different systems theories. “A system is a collection of independent but interrelated elements or components oraganized in a meaningful was to accomplish an overall goal” (Hayajneh, 2007, p. 1). In order for an organization to be run successfully there must be a system theory in place.
