NURS 6531 Week 3 Assignment 1 SOAP Note Hypertension
In clinical settings, advanced practice nurses frequently use various strategies to treat and manage patients with hypertension and other cardiovascular disorders. These strategies often include pharmacologic and nonpharmacologic therapies, natural remedies, and/or changes in patient behavior. For hypertension patients, behavioral changes including increased exercise, healthier diet, and smoking cessation have proven to be particularly beneficial. However, it is important to recognize that treatment and management plans centered around changes in behavior often require greater patient commitment. This creates the need for patient-provider collaboration, as well as appropriate patient education. When patients are actively involved in their own care and better understand implications of their disorders, they are more likely to adhere to treatment plans.
Review Part 11 of the Buttaro et al. text and the National Heart Lung Blood Institute article in this week’s Learning Resources.
Reflect on your Practicum Experiences and observations. Select a case from these experiences that involves a patient who presented with a hypertension problem. When referring to your patient, make sure to use a pseudonym or other false form of identification. This is to ensure the privacy and protection of the patient.
Think about the patient’s history including drug treatments and behavioral factors such as diet, exercise, smoking, etc.
Review the National Heart Lung Blood Institute article in the Learning Resources. Reflect on health promotion strategies for the patient. Consider ways to reinforce hypertension management.
Post on or before Day 3 a description of a patient who presented with a hypertension problem during your Practicum Experience. Explain the patient’s history including drug treatments and behavioral factors. Then, suggest two health promotion strategies for the patient. Theory: Self Care
As a nurse transitioning into a primary care provider, my roles will change significantly. Although my roles will change, one thing that will not change will be my duty to educate and promote self-awareness and self-care. As healthcare providers, we are to encourage our patients and their family to be active in their care. Throughout this journal, Dorothea Orem’s Self-Care Deficit Theory will be discussed and illustrated how this theory would guide my personal practice.

As I begin my practicum, I believe it is important to familiarize myself with and utilize established nursing theories to maximize the experience. As I pondered on which theory to use I thought about what theory I would want a healthcare provider to use as they are caring for me. I feel that Paterson & Zderad’s Humanistic Nursing Theory will assist me and guide my practicing. It portrays the potential to mold me into the practitioner I hope to become. The Humanistic Nursing Theory rose up out of Paterson’s and Zderad’s desire for an approach to improve things for healthcare professionals and their patients as they interacted with and performed the day to day activities of nursing practice (Parker & Smith, 2010). Week 1 – Establish a relationship with preceptor, develop an understanding of the expectations of the clinical site, observe basic workflow and population of patients seen and set realistic and achievable goals with preceptor.
Week 2 – Demonstrate the ability to adequately communicate with preceptor and other members of the health care team at clinical site.
Week 3 – Begin to effectively demonstrate the ability to perform a physical assessment on patients and document findings within the electronic medical record.
Week 4 – Demonstrate the capacity to successfully evaluate patient side effects, determine the conceivable intercessions required and apply those indications to justifications for interventions.

Over time, this disorder will become progressively worse, and patients will eventually experience a loss of renal function. Early detection and prevention is key for patients with this disorder. For this reason, it is important for you, as the provider, to be aware of various signs, symptoms, and risk factors of chronic kidney disease.
