FAQ

When I submit my work, why is the time posted different than the time zone where I live?
The servers that host your online courses are housed in the Eastern Time zone. The time settings cannot be changed. Course work however is due by 11:59 PM Mountain Time zone. This often happens when the document is saved in a format that your computer is not able to read. Having your classmate re-save and re-post the document in Rich Text Format can alleviate this problem. This format should be readable by all Mac and Windows computers. You may need to e-mail the owner of the document and ask her/him to re-save it in this format. You can tell what format the document is currently saved in by looking at the extension after the saved name. (This will be the part of the name of the file that appears after the period.) If the file is in rich text format or text format, it will read “.rtf” or “.txt” respectively.

The pediatric behavioral and mental health (PBMH) specialty addresses the mental health care crisis that threatens children and adolescents across our country. As one of the first pediatric nursing specialty program of its kind in the nation, we aim to improve care by equipping you with specific pediatric behavioral and mental health care knowledge, skills and clinical practice experiences.

The PBMH specialty offers you the opportunity to gain advanced training in pediatric primary care as well as behavioral and mental health assessment and treatment, diagnosis and management.

Coursework and clinical experiences prepare you to serve in a variety of settings ranging from primary care practices, school-based health centers and pediatric specialty clinics as well as skills to address the healthcare needs in patients located in rural or underserved communities.

Enrollment Options

The pediatric behavioral and mental health specialty is available to our students who are preparing to become certified as a Pediatric or Family Advanced Practice Nurse (APRN).

The three-course specialty sequence is designed to educate emerging APRNs to provide skilled behavioral and mental health assessment, diagnosis and management in a variety of pediatric clinical settings and prepare graduates for Pediatric Mental Health Specialist certification. Certified APRNs who are currently practicing in clinical settings may wish to complete the PBMH Management I course (rather than the complete specialty sequence) to meet the preparation requirement for the Pediatric Mental Health Specialist certification. If you are interested in adding this specialty to your academic plan, you must complete the Add a Specialty Form. This form must be submitted to the MSN Program Office via email (son‐msn@dm.duke.edu) at least 10 days prior to the start of the semester. Nurse practitioners with an MSN degree or higher from a regionally accredited institution and nurse practitioner students from other regionally accredited colleges or universities interested in taking any of the courses in this sequence, should contact Duke University School of Nursing Admissions via email (SONAdmissions@dm.duke.edu) to register as a non-degree seeking student.

Certification

The pediatric behavioral and mental health specialty also prepares graduates for the Pediatric Primary Care Mental Health Specialist certification exam offered by the Pediatric Nursing Certification Board. Graduates will have to complete a minimum of 2,000 clinical hours before they are eligible to sit for the exam. Q: Is the equipment and/or software expensive?

Depending upon the circumstances by which telepsychiatry is introduced to an organization’s culture; staff resistance may present a challenge. A: The practitioner must be licensed in the state where the patient is physically located and receiving services. Currently 29 states have joined the interstate compact which allows registered nurses to provide care in any of the participating states as long as one of the compact states has issued the individual an RN license with multi-state privileges. States which do not participate therefore require APRNs to obtain both the RN license and APRN license prior to delivering clinical care.

Most states do not have a specific telemedicine license and require a full license in order to provide services to patients receiving services in those states. Several states have very difficult licensing requirements which may result in greater delays to rendering services if we do not currently have a practitioner available in that state.

Q: Will Medicare, Medicaid and private insurance companies pay for telepsychiatry?
A: Medicare reimburses for psychiatric evaluation, medication monitoring and several other services as long as the patient site is located within a Health Professional Shortage Are (HPSA). Q: Does FasPsych provide APRN supervision if my state requires this?
A: FasPsych practitioners work at an independent level however they are instructed to follow the chain of command at your organization. Ultimately, with regard to clinical or patient site administrative protocol, they report to your medical director, or the highest ranking clinical administrator.

Practicum Activities

The practicum experience in this course will assist your transition from the role of learner to that of scholar-practitioner. To achieve this transition, you will engage in a relationship with a clinical instructor and preceptor, focusing on roles and role functions and the achievement of individualized learning objectives. The primary objective of your practicum is to provide you with the basic skills necessary to serve as a mid-level provider of primary care to selected populations and prepare you to take the appropriate national certification exam. The practicum component of the course will be graded as Satisfactory or Unsatisfactory. In order to pass the course, you must earn a grade of Satisfactory on all required practicum activities including journal entries and time logs.

At the end of the course, you must also ensure that your practicum preceptor submits an online evaluation of your performance. You will also complete an online evaluation of your practicum experience at the end of the course. These evaluations will not only provide information about the progress of individual students, but also help program leadership to continuously work on the course review and improvements. Time Logs: Students are required to keep a log of the time spent related to their practicum experience and enter every patient they see each day. Students will identify examples of imagery and figurative language in passages from the text and will interpret the ideas they express. Introductory Lecture:

In an 1837 address at Harvard University, Ralph Waldo Emerson famously declared that the United States had too long imitated the ideas and outlook of the old world. The time had come for American poets and intellectuals to speak and act more like themselves and less like Europeans.

Emerson’s speech, now called “The American Scholar,” forcefully argued that authentic American expression would appear when writers turned away from the past for inspiration and looked instead at the wonder and beauty of the natural world around them.

With his 1850 book Walden, Emerson’s protégé Henry David Thoreau appears to take on Emerson’s challenge wholeheartedly. It is a detailed exploration of Emerson’s idea that the United States would find its own cultural and literary identity and escape the burden of European influence through an intimate and unfiltered study of nature. The book chronicles the two years Thoreau spent living in a simple one-room cabin next to Walden Pond in Concord, Massachusetts. At Walden Pond, Thoreau endeavored to remove from his life all but the bare essentials, foregoing luxuries, eating mostly vegetables, and generally pinching pennies. Free to spend most of his hours observing and contemplating the natural world, he found he was rewarded with deep self-knowledge and spiritual riches that could not be found in books or the religions of the past. In Walden, Thoreau uses elaborate imagery to show how he immersed himself in the seasons, sounds, moods, and colors of Walden Pond. At times he explains the spiritual lessons he takes from his observations, but much of the time he leaves it to readers to glean deeper truths in his detailed natural imagery. The imagery in “The Ponds,” one section of the book, is rich in the optical effects and mysterious sounds of the pond, revealing Thoreau’s love of the reflections, echoes, shifting colors, and delicate surface patterns that transport him to spiritual realms and invite mystical interpretations. While drifting in his boat on a still day, for instance, the glassy surface of the pond reflecting the sky and clouds, he experiences a disorienting and elating sensation of floating in the sky. Such is the power of nature, Thoreau suggests—to transform our reality, once we learn to truly see and hear its splendor. The R.N. to B.S.N. is an online program leading to a Bachelor of Science in Nursing (B.S.N.). The program is designed for licensed R.N.s who have either an associate degree or diploma in nursing.
