Assessment 3. Case study essay (50%) – Guidelines

Please refer to the three patient case studies (A, B and C) provided on FLO for this assessment.

If your surname begins with A – M Please use case B as a basis for this assessment

If your surname begins with N -Z Please use case C as a basis for this assessment.

While each case study is not fully comprehensive, you are to use information provided in the case study ONLY, do not include additional information. You may however make some assumptions based on the information provided in the case studies.

The assignment: NURS 3003 Assessment 3 – Dynamics of Practice

Provide a 3,000 word essay that includes:

  • an introduction in which you introduce your case study patient and provide context for what is to follow in the essay.
  • Illustrate what you understand by the terms chronic and complex needs?.
  • Do you believe that your case study patient has complex care needs? If so, why?
  • identify in order of priority, three (3) specific needs for your patient.
  • Provide a rationale as to why you have prioritised these needs
  • For each of these needs, provide a detailed account of how they will best be met (in the patients home). This will include information about how these needs will be met and the multidisciplinary team members required, their role and a justification as to why they will be beneficial in assisting the persons care and avoiding hospitalisation.
  • demonstrate the role of the registered nurse in assisting the patient to access services.
  • Identify two potential barriers to this care, this may include issues such as social capital, ageism, cultural and social processes if applicable.
  • Consider any issues related to the patient’s medication needs. (actual or potential)
  • Provide a conclusion that sums up the main points in your essay.

Additional considerations for NURS 3003  Assessment 3 – Dynamics of Practice:

This is a lengthy assessment and will require a lot of planning to ensure that you cover the asked for elements. In planning your essay, please refer to the marking rubric. You are encouraged to use sub-headings throughout to assist the reader and focus your discussion.

You are to assume that the patient is returning to home (in the first instance) following their recent hospitalisation.

It is expected that you include a variety of reference sources including government reports, credible government sites where appropriate and at least four (4) evidence based peer reviewed journal articles as well as any others you choose to include.

You are expected to provide references that comply with the required APA 6th Edition referencing style.

CASE B Patient:

Mrs Mary Tonkin DOB: 06/06/1940 Weight: 63 kgs Height: 168 cms Family/Significant others Three grown sons, two live in Queensland and her youngest son Sam lives with his wife and their 4 young children in a suburb 20 minutes drive from Mary. Her son Sam receives a carers allowance to care for his mother and is very worried about her. He is supportive and visits her three times a week. Rachael, his wife is unhappy with Sam spending so much time at Marys house as she is busy working full time and caring for the children. Rachael would like Mary to be placed in a residential aged care facility, however Sam knows that this is not what Mary wants.

Background

Mrs Mary TONKIN is a 79 year old widow who lives alone in a house in the suburb of Marion in metropolitan South Australia. Marys husband died 10 years ago following a fall from a ladder and a fractured femur. Complications contributing to his death were osteomyelitis and pneumonia. Consequently, Mary is fearful of hospitals and is adamant that she wants to stay in her own home. Mary lives (with her two cats) independently in the large family home. Over the past three years she has noticed her health declining which has made it difficult for her to carry out activities around the home such as cleaning and gardening as she is becoming very tired and occasionally, diaphoretic and short of breath. Mary also feels as though she is becoming more forgetful lately as she found herself placing frozen food items in the cupboard rather than the fridge. Mary has some left sided leg hemiparesis following her recent stroke but manages to walk with the aid of a walking stick. Hospital stay Mary has spent the last five days in hospital recovering after an episode of dizziness, shortness of breath and fatigue.

Assessment 2. Explanation paper (30%) – Guidelines
Use the information sheet that you have developed for assignment 1 as a basis for this assignment.
This assignment asks you to provide rationales for the information you have provided in your information sheet (Assignment 1).
The assignment:
Provide a 1,500 word essay that illustrates:
1. Your rationale for selecting the four (4) identified needs for your case study person/family/carer/s identified in your information sheet.
For each of these needs, (chosen from the Commonwealth Home Support Programme) provide a rationale explaining the relevance of this need to your allocated case study person. i.e. why have you identified these needs as being important for the person?
2. Provide a rationale for the priority order in which you illustrated the four needs
For example, why have you prioritised the needs in the way you have, i.e. the first and most important, through to the fourth and least important
3. Your rationale for identifying the services linked to your identified needs.
For each of the identified services, provide a rationale explaining the benefits of the identified services to the person/family/carer/s. Who might be involved in providing this care and how can they assist in hospital avoidance for your allocated person?
4. A rationale that explains the design of your information sheet.
Why did you choose to include or not include any images, the font size, colour/s, design, layout etc.
