The primary focus of a community health nurse is to improve the health of the community. To do this involves using demographic and epidemiological findings to assess the community’s health and diagnose its needs. This assessment is aligned with the module objective “Discuss the elements of practice in community/public health nursing.”

This assignment is not a paper, so there is no cover page. All of the work is completed by completing the attached template. As part of the assignment, you will complete the following tasks: typing data, an analysis of the findings with four (4) references, and community health nursing diagnoses directly on the template.

· Compile a range of relevant demographic and epidemiological data found on the community assessment rubric for this assignment. Use the websites listed below, as well as other websites (you can also do a Google search to find the health data you are looking for), gather information about your county and compare it with your state and national numbers.

· This assignment is not intended to be overwhelming and should take no more than 3-4 hours to complete.

· If you are unable to locate data, check with your peers to see where their information was located or you may record “no data available” in the provided assessment template.

· Record the most recent data available; the data you record should be dated within the last 5 years;

· Complete the assignment using the attached template on STEP 2.

· Once you have obtained the required data, in the Analysis Section of the template, write a summary of findings in the space provided. Click on the analysis space and begin typing your summary. Identify actual and at risk health problems from the data that are relevant to your community.

· Below the analysis summary formulate 2 (two) community health diagnoses using the data you have collected. Use the Figure 6-3 format in Chapter 6, pp.103 of your textbook. Please check the textbook because the table is not coming up on the word doc. I will attach a photo.

· How your community is doing in comparison with other communities?

This assignment is worth 40 points of your final grade.

NOTE: This assignment is about the experience of using databases to locate information about your community. Keep in mind as you complete this assignment that you may not be able to locate all data fields listed in the worksheet.

Record the data you collect into the following tables. Record the source (website) of your data in the Source Column. Collect the most recent data available (usually within the last 5 years). If you cannot find that data for a few inquiries just enter “NDA” – No Data Available. However, this should be limited to two or three areas.

Public Services and Access to Care

Data MUST be presented as number per 1000 people or in a comparable manner (i.e. percentage by population)

Analysis:

Reflect on the ways the information you obtained relates to the county where you live and the Healthy People 2030 National Health Objective you have selected to study.

In addition, write two priority community health nursing diagnoses . Use the correct format for your diagnoses: Problem, Population or location of problem, Etiology, Signs/Symptoms (***Chapter 6, pp. 102 in your textbook).

Formatting would be: Problem related to the etiology as evidenced by the signs/symptoms.

For example: Risks of asthma complications among children in a southeast Ohio County related to poor air quality, overcrowded living conditions, inability to access health care, high number of animals living in homes, lack of knowledge regarding treatment options, and lack of access to medications as evidenced by increasing numbers of hospitalizations due to asthma complications.
