Locate your state’s nurse practice act (NPA) and associated regulations governing prescribing by advanced practice nurses (CNPs, CRNAs, CNMs, CNSs). Answer and discuss the following in this forum:
- Does your NPA require the APRN to have a collaborative agreement with a physician? Discuss whether you think the NPA should or should not require the APRN to have a collaborative agreement, and explain why/why not.
- Does your NPA require the APRN to have a prescribing agreement with a physician? Discuss whether you think the NPA should or should not require the APRN to have a prescribing agreement, and explain why/why not.
- Does your NPA permit APRNs to prescribe all classes (schedules) of medications? Discuss whether you think the NPA should or should not permit APRNs to prescribe all classes of medications, and explain why/why not.

Example Discussion Solution Approach
In the state of Rhode Island, APRNs are not required to have a collaborative agreement with a physician. APRNs are licensed by the State Board of Nursing and they are authorized to practice their profession independently. In state policy, APRNs are acknowledged as primary care providers. According to § 5-34-3, Advanced practice registered nurses have the autonomy and a broad scope of nursing practice that enables them to serve a population focus that is approved by the board of nurse registration and nursing education allowing them to perform an advanced assessment, diagnosing, prescribing, and ordering (NCSLSOP, 2021). I believe APRNs should be able to practice fully without a collaborative agreement with an MD, as long as they are fully licensed to do so. The APRNs have both the specialized advanced education and clinical skills required to provide medical care to a diverse population in different areas of primary care settings.
APRNs are able to prescribe any medications that an MD can prescribe in the state of Rhode Island. “Prescriptive privileges for the certified nurse practitioner shall include all the authority under the APRN license including Prescription of legend medications and prescription of controlled substances from Schedules II, III, IV, and V that are established in regulation..” (NCSLSOP, 2021). APRNs are certified registered nurse practitioners and advanced practice nurses who have acquired adequate knowledge of physical assessment and management of healthcare and illnesses, and therefore should have the same prescriptive privileges as the physician. APRNs receive their prescribing privileges upon completion of pathophysiology, pharmacology, and physical assessment, as long as it is within the population domain of their practice (NCSLSOP, 2021). In Rhode, APRNs can prescribe all classes of medication that an MD is able to prescribe. Per Rhode Island law, APRNs have full authority to prescribe “legend drugs and Schedules II – V controlled substances if within the APRNs role and population focus”(NCSLSOP, 2021). I believe APRNs have the advanced knowledge and clinical competence required to proscribe all classes of medications within their scope of practice.
The goals of the treatment regimen for rashes across the continuum include replacing the original integrity of the outer layer of the skin, treating skin irritation, and managing the itch. The treatment of choice are corticosteroids. If the rash is diffuse, systemic corticosteroids are recommended, and antihistamines are usually the treatment of choice for extreme itch management. If the rashes are mild to moderate, topical corticosteroids are normally utilized two times a day for a period of two weeks (Arcangelo, et al., 2017). “As a rule, short-term therapy with more potent topical corticosteroids is preferred to long-term therapy with less potent corticosteroids.” So, If symptoms persist after 2 weeks of the above treatment regimen, then CNP can collaborate with other consultants for more potent effective treatment options.
